MONTHLY SWEEPSTAKES WILL BEGIN AT 12:00 P.M. ON THE FIRST CALENDAR DAY OF EACH MONTH FROM AUGUST 1, 2012 AND WILL END AT 11:59 P.M. ON JULY 31, 2013 ("SWEEPSTAKES PERIOD"). ALL TIMES REFERRED TO IN THESE OFFICIAL RULES ARE CENTRAL STANDARD TIME (CST) OR CENTRAL DAYLIGHT TIME (CDT) DURING DAYLIGHT SAVINGS TIME.

How to Enter

Entrants and Winners must be legal U.S. residents of one of the following Illinois counties: Cook, DuPage, Kane, Lake, McHenry, Will, Grundy or Kendall. Entrants and Winners must be 18 years of age or older at time of entry with the ability to provide proof of Illinois residency with valid identification. Odds of winning depend on number of entries received. Odds of winning a prize will decrease for entrants as Sweepstakes approaches its monthly end dates.

There are three ways to enter the Breathe Easy Save Easy Sweepstakes (the "Sweepstakes"):

Online Entry: To enter online, log onto http://www.breatheeasysaveeasy.com, or find a link to the Sweepstakes on www.cleantheair.org. Click on the Sweepstakes link and complete the online air quality survey and online entry form. Only one email address per individual may be used for the Sweepstakes; individuals using multiple email addresses risk disqualification of all entries. The entry form will ask individuals to verify that they are at least 18 years of age at time of entry.

The online entry form will ask individuals to answer questions regarding voluntary actions to reduce air pollution and save money prior to completing the entry form for the Sweepstakes. These responses will be used by the Administrator to gauge public activities in relation to air pollution reductions. The online form will also ask entrants to indicate where they learned about the Sweepstakes and will allow them to mark checkboxes to receive additional information. To unsubscribe from the email list, send an email to the following address: pfca@cleantheair.org. The online form will also allow entrants to check a box to sign-up to receive Air Pollution Action Day notices through a free Illinois EnviroFlash subscription. Entrants who opt to subscribe to receive these notices will receive a second entry in their name for the given month. Only new Illinois EnviroFlash subscriptions will be eligible for a second entry for the given month. REGISTERING AS A NEW SUBSCRIBER FOR ILLINOIS ENVIROFLASH IS THE ONLY WAY TO RECEIVE TWO ENTRIES INTO THE MONTHLY SWEEPSTAKES. To unsubscribe from Illinois EnviroFlash emails, click the UNSUBSCRIBE link at the bottom of the email. To be eligible for the monthly prize all online entries must be received before 11:59 pm on the last calendar day of each month (August 31, 2012, September 30, 2012, October 31, 2012, November 30, 2012, December 31, 2012, January 31, 2013, February 28, 2013, March 31, 2013, April 30, 2013, May 31, 2013, June 30, 2013, July 31, 2013,) to be eligible for the Sweepstakes. ONLY ONE ENTRY IS PERMITTED PER PERSON DURING EACH MONTH OF THE SWEEPSTAKES REGARDLESS OF METHOD OF ENTRY, UNLESS REGISTERING AS A NEW SUBSCRIBER FOR ILLINOIS ENVIROFLASH.
On-Site Entry: To enter on-site, you may complete an official Breathe Easy Save Easy Sweepstakes entry form at various special events throughout the Sweepstakes Period. Sweepstakes representatives will be available on-site at select special events to accept completed entry forms. Dates and events at which entry forms will be available are subject to change at the Administrator's sole discretion. Administrator is not liable if entry forms are not available on any given day at any given event. To find out about current events where entry forms will be available, go to http://www.cleantheair.org/calendar. Individuals may complete the entry form on site, which will be collected as an official entry by the Sweepstakes representative, or individuals may mail the entry form to: Breathe Easy Save Easy Sweepstakes, c/o Illinois EPA, 1021 North Grand Avenue East, #17, Springfield, IL 62702. All mail-in entries must be hand-printed and legible. Mailed entries must be postmarked no later than the last calendar day of the month in which each prize is being awarded with the exception of three months (September, March and June) where the last calendar day of the month is a Sunday. In these three instances, mailed entries must be postmarked no later than the last calendar Saturday of the month in which each prize is being awarded. Latest postmark dates for each month are as follows: August 31, 2012, September 29, 2012, October 31, 2012, November 30, 2012, December 31, 2012, January 31, 2013, February 28, 2013, March 30, 2013, April 30, 2013, May 31, 2013, June 29, 2013, July 31, 2013. In addition to the postmark requirements, mailed entries must be received no later than the close of business, five business days after the last calendar day of the month each prize is awarded: September 7, 2012, October 5, 2012, November 7, 2012, December 7, 2012, January 7, 2013, February 7, 2013, March 7, 2013, April 5, 2013, May 7, 2013, June 7, 2013, July 8, 2013, August 7, 2013, to be eligible for the Sweepstakes. The Administrator, the Agency and the Sponsor are not responsible for lost or misdirected mail. No photocopies or mechanical reproductions of entry forms are permitted. ONLY ONE ENTRY IS PERMITTED PER PERSON DURING EACH MONTH OF THE SWEEPSTAKES REGARDLESS OF METHOD OF ENTRY, UNLESS REGISTERING AS A NEW FREE SUBSCRIBER FOR ILLINOIS ENVIROFLASH.
Mail-in Entry: To enter by mail without an official Breathe Easy Save Easy Sweepstakes entry form, print your full name, complete home mailing address (no Post Office Boxes) including zip code, county of residence, daytime phone number and birth year on a 3x5 postcard and mail it to: Breathe Easy Save Easy Sweepstakes, c/o Illinois EPA, 1021 North Grand Avenue East, #17, Springfield, IL 62702. All mail-in entries must be hand-printed and legible. Mailed entries must be postmarked no later than the last calendar day of the month in which each prize is being awarded with the exception of three months (September, March and June) where the last calendar day of the month is a Sunday. PLEASE NOTE: All eligible entries, including those received via U.S. mail, are added to an electronic database. Mail-in entries will be entered by the date they are postmarked. ONLY ONE ENTRY IS PERMITTED PER PERSON DURING EACH MONTH OF THE SWEEPSTAKES REGARDLESS OF METHOD OF ENTRY UNLESS REGISTERING AS A NEW SUBSCRIBER FOR ILLINOIS ENVIROFLASH. Winners are randomly selected from this electronic database. Entering by mail will in no way increase the chances of winning.

MULTIPLE ENTRIES PER MONTH WILL NOT BE ACCEPTED WITH THE EXCEPTION OF NEW SUBSCRIBERS FOR ILLINOIS ENVIROFLASH. Sweepstakes entries are checked regularly to ensure that entrants are not violating this rule. In the event an entrant is found to have submitted more than one entry during each Sweepstakes month, the Administrator will disqualify any and all entries submitted by that entrant for the entire Breathe Easy Save Easy Sweepstakes. The Administrator reserves the right to disqualify entries for the violation mentioned above without any obligation to notify the entrants of their disqualification.

Incomplete entries will be disqualified.

Eligibility

The Sweepstakes is open to persons who are eighteen (18) years of age or older at time of entry. Entrants and Winners must be legal U.S. residents of one of the following Illinois counties: Cook, DuPage, Kane, Lake, McHenry, Will, Grundy or Kendall.

Employees of Illinois Environmental Protection Agency (the "Agency") and any of their respective affiliates, subsidiaries, volunteers, promotion or advertising agencies or any other company involved with the design, production, execution or distribution of the Sweepstakes and immediate families or household members of each of the foregoing are ineligible to enter. "Household member" shall mean people who share the same residence at least three (3) months a year. Void where prohibited. All federal, state, and local laws and regulations apply.

Selection of Monthly Prize Winners

Throughout the Sweepstakes Period, one (1) entry will be randomly selected from among all eligible entries for each Month (August 2012 - July 2013 as defined above) to win the Monthly Prize specified for each given month. A representative of the Agency will contact the Winner(s) based on information provided on the submitted entry. Monthly Prize packages will be sent via United Parcel Service or United States Postal Service to the address provided by the Winner(s). If the Agency is unable to contact Winner(s) from the information provided after a reasonable effort has been made, it reserves the right to select another Winner(s) at its sole discretion. All decisions of the Administrator and Agency are final and binding on all matters relating to the drawing and the Sweepstakes generally.

Only one (1) Monthly Prize per household for the duration of the Sweepstakes.

Monthly Prize(s)

Throughout the Sweepstakes Period, one (1) winner will be randomly selected from all eligible entries received to date for each month. The person(s) selected will win a Monthly Prize, which will be delivered to the address listed on the entry. The Monthly Prize(s) will be an environmentally conscious prize or package valued at approximately $200. Monthly Prizes may contain items such as museum passes with a public transportation voucher. This description is for illustrative purposes only; winners may receive different prizes to be chosen by the Administrator in its sole discretion.

Winners agree that the Administrator, Agency, and Sponsor involved with Sweepstakes have neither made nor are responsible for any warranty, representation, or guarantee, expressed or implied, in fact or in law, relative to the prizes, including, but not limited to their quality, condition, or fitness.

General

By entering, entrants agree to be bound by and acknowledge compliance with these Official Rules including, without limitation, eligibility requirements and the decisions of the Administrator, Agency, or Sponsor. In the event any prize is unavailable for any reason, the Administrator, Agency, or Sponsor reserves the right to substitute a comparable prize of equal or greater value. Prizes may not be assigned, substituted, transferred, changed or redeemed for cash. Failure to comply with these Official Rules will result in disqualification. Taxes on prizes, if any, are the sole responsibility of Winners.

Winners must sign and return Affidavit of Eligibility and Publicity Release within five (5) business days after receipt. Failure of any Winner to return the executed Affidavit and Release form(s) within the specified time period, non-compliance with these Official Rules or return of the Sweepstakes notification as undeliverable will result in that Winner's disqualification and forfeiture of the prize, and an alternate Winner(s) will be selected from remaining eligible entries for the applicable drawing.

Release

By accepting the prize, the Winners each:

release Illinois Partners for Clean Air c/o the Illinois Environmental Protection Agency (the "Administrator"), Illinois Environmental Protection Agency (the "Agency"), Illinois Partners for Clean Air (the "Sponsor"), and their respective officers, directors, stockholders, affiliates, agents, employees, advertising agencies, and all others associated with the development and execution of this Sweepstakes from and against any and all liability with respect to or in any way arising from this Sweepstakes and/or acceptance or use of any prizes, including, without limitation, liability for any injury, damage, loss, death, expense, accident, delay, inconvenience, or irregularity arising out of or relating to any prize granted under this Sweepstakes, even if caused by the negligence of the Sweepstakes Administrator, Agency, and Sponsor; and
consent to use of their names, voices, photographs, testimonials and other likeness and/or prize information or personal exposition (and/or any edited portion thereof) for promotional, advertising, publicity and/or trade purposes in any and all media, now or hereafter known throughout the world in perpetuity by the Administrator, Agency, and Sponsor without further compensation or notice to, or further consent of, to the Winner to the extent permitted by law. Any person who supplies false information, obtains entries by fraudulent means, or is otherwise determined to be in violation of these rules in an attempt to obtain any prize will forfeit any prize won and will be prosecuted to the fullest extent of the law. All applicable parties including the Administrator, the Agency, and the Sponsor are not responsible for any typographical errors in the printing of these Official Rules, administration of the Sweepstakes or in the announcement of the Sweepstakes prizes.

Internet Disclaimer

If the Internet portion of the program is not capable of running as planned for any reason, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Administrator which corrupts or affects the administration, security, fairness, or integrity of the Sweepstakes, the Administrator and/or Agency reserves the right in its sole discretion to disqualify any individual believed to have tampered with the entry process, and/or to cancel, terminate, modify or suspend the Sweepstakes. The Administrator assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries. The Administrator is not responsible for any problems or technical malfunction of any telephone network or lines, computer online systems, servers, or provider, computer equipment, software, failure of any email to be received by the Administrator on account of technical problems, human error or traffic congestion on the Internet or at any Web site, or any combination thereof, including any injury or damage to the entrant's or any other person's computer related to or resulting from participation or downloading any materials in this Sweepstakes. The use of automated entry devices is prohibited. CAUTION: ANY ATTEMPT TO DELIBERATELY TAMPER WITH THE SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, THE ADMINISTRATOR RESERVES THE RIGHT TO SEEK DAMAGES AND OTHER REMEDIES FROM ANY OFFENDER TO THE FULL EXTENT PERMITTED BY LAW.

In the event of a dispute over the identity of an online entrant, the entry will be deemed to have been submitted by the authorized account holder of the email account from which the entry is made. The "authorized account holder" is the natural person to whom an email address is assigned by an Internet access provider, online service provider or other organization (e.g., business, educational institution, etc.) responsible for assigning email addresses for the domain associated with the submitted email address.

Delivery Disclaimer

The Administrator, Agency, and Sponsor disclaim all liability for any delays, misdelivery, loss, or failure in the delivery of any items sent by mail, courier, express, electronic transmission, or other delivery method.

Compliance with Law

The conduct of the Sweepstakes is governed by the applicable laws of the state of Illinois, which take precedence over any rule or law to the contrary herein.
